Job Description

Join Our Compassionate Team as an Occupational Therapist (OT) At Quality Care Home Health & Hospice, we're dedicated to providing exceptional care that empowers patients to live more independently and comfortably. We're seeking a skilled Occupational Therapist (OT) to join our team, where you'll have the opportunity to make a meaningful impact on the lives of individuals and families in both hospice and home care settings.
Your Role and Responsibilities:
As an Occupational Therapist, you will:
Assess Needs:
Conduct comprehensive evaluations of patients' functional status and home environment, identifying equipment needs.
Create Care Plans:
Develop and communicate personalized treatment plans, coordinating with physicians and care teams.
Enhance Daily Living:
Teach and retrain patients in daily activities, adaptive equipment use, and prosthetics/orthotics.
Support Hospice and Home Care:
Provide specialized care tailored to both hospice and home care patients.
Document and Report:
Maintain accurate clinical records and report patient condition changes.
Coordinate and Supervise:
Ensure efficient care through communication with team members and supervision of therapy assistants.
Educate and Train:
Instruct patients, families, and caregivers on treatment regimens and therapeutic techniques. Participate in
Care Planning:
Contribute to team meetings and assist in discharge planning.
What We're Looking For:
We're seeking an Occupational Therapist who is not only experienced but also passionate about improving patients' quality of life.
The ideal candidate will have:
A degree from an accredited occupational therapy program. Registration with the National Board for Certification in Occupational Therapy (NBCOT). At least two years of clinical experience as an occupational therapist, with experience in both hospice and home care preferred. Strong verbal, written communication, and organizational skills. Current CPR certification. A valid driver's license and a reliable, insured vehicle.
