293417011 Job Description Description This is an on call/per diem position that will be supporting all western WA PACE sites. The Occupational Therapist (OT) is responsible for OT evaluations and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Bachelor's Degree in Occupational Therapy Within 30 days of hire: National Provider BLS•American Heart Association Upon hire:Washington Occupational Therapist License Preferred Qualifications:
Master's Degree in Occupational Therapy, or Ph.D. in Occupational Therapy 1 year experience working as an occupational therapist in a geriatric setting.
