Part-Time or PRN Occupational Therapist (In-House Rehab Team) Cleveland Community Care Center Cleveland Community Care Center is seeking a Part-Time or PRN Occupational Therapist to join our in-house rehabilitation team . This is a great opportunity to work in a collaborative, resident-centered environment where clinical quality, consistency, and compassionate care are highly valued. As part of our in-house therapy department , you will work alongside an experienced interdisciplinary team and have the opportunity to deliver meaningful, patient-focused care—without the pressures often associated with contract therapy models. Position Highlights Part-Time or PRN Occupational Therapist In-house rehabilitation program Collaborative and supportive clinical team Opportunity to positively impact resident independence and daily function Flexible scheduling options Key Responsibilities Evaluate residents and develop individualized occupational therapy treatment plans Provide skilled occupational therapy interventions in a skilled nursing setting Document treatments and progress in accordance with regulatory and facility standards Collaborate with nursing staff, physicians, and interdisciplinary team members to support resident goals Participate in care planning and discharge planning meetings Promote quality outcomes while maintaining ethical, evidence-based practice If you are an Occupational Therapist who values teamwork, flexibility, and a supportive therapy environment , we would love to connect with you about joining the team at Cleveland Community Care Center .
