Location - Winchester , VA Summary As an Occupational Therapist at 1st Americare LLC, you will play a vital role in enhancing the lives of our clients through tailored therapeutic interventions. This position is essential in supporting our commitment to delivering compassionate and professional home health care. Responsibilities Conduct comprehensive assessments of clients' needs and develop individualized treatment plans. Implement therapeutic interventions to improve clients' daily living skills and overall quality of life. Collaborate with a multidisciplinary team to coordinate care and ensure continuity of services. Educate clients and their families about treatment plans and strategies for maintaining independence. Monitor client progress and adjust treatment plans as necessary to achieve optimal outcomes. Document client progress accurately in compliance with regulatory standards. Participate in discharge planning to facilitate smooth transitions for clients leaving skilled facilities or hospitals. Provide training and support to caregivers on therapeutic techniques and client care. Qualifications Valid Occupational Therapist license in the applicable state(s). Experience in a skilled nursing facility or home health care setting preferred. Strong knowledge of medical terminology and case management practices. Ability to develop effective treatment plans based on client assessments. Excellent communication skills for interacting with clients, families, and healthcare professionals. Understanding of physiology and its application in occupational therapy interventions. Experience with early intervention strategies is a plus. Flexibility to adapt to various client needs and environments.
