Occupational Therapy Assistant, Outpatient at Home Sun City, AZ Full-time, PRN, Contract Full-time, PRN, Contract Overview Join our dynamic team as an Occupational Therapy Assistant in our home based outpatient therapy practice, where you will play a pivotal role in enhancing the quality of life for patients in their own environments! In this rewarding position, you will provide compassionate and expert therapy services to individuals recovering from falls, injuries, surgeries, or managing chronic conditions. You'll have the opportunity to develop personalized care plans that empower patients to regain their independence and improve their physical well-being. What you'll do Follow comprehensive POC developed by supervising Occupational Therapist Utilize your knowledge of anatomy and physiology to educate patients about their conditions and the rehabilitation process. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure continuity of care. Maintain accurate documentation using electronic medical record (EMR) systems to track patient progress and outcomes. Foster a supportive environment that encourages patient motivation and adherence to therapy regimens. Basic qualifications Valid Occupational Therapy Assistant license in the state of Arizona. Strong understanding of medical terminology and therapy practices. Preferred qualifications Experience in home health, outpatient clinics, or hospital settings. Knowledge of discharge planning processes and community care resources. Proficiency with EMR systems for efficient patient management.
