Outpatient Physical Therapist

Job Overview We are seeking a dedicated and energetic Outpatient Physical Therapist to join our newly established outpatient-ortho Physical Therapy Clinic. In this vital role, you will assess, develop, and implement personalized physiotherapy treatment plans to improve patients' mobility, strength, and overall well-being. You will work closely with a diverse patient population, including orthopedic, geriatric, and sports specific cases, providing expert care in outpatient clinic settings. Your expertise will help patients regain independence and enhance their quality of life through targeted therapy interventions. Duties Conduct comprehensive patient evaluations to determine physical therapy needs, utilizing knowledge of anatomy, physiology, and medical terminology Develop individualized care plans that incorporate evidence-based practices such as trigger point therapy and orthopedics techniques Administer therapeutic interventions including manual therapy, exercise programs, and modalities tailored to each patient's condition Monitor patient progress regularly and adjust treatment strategies to optimize outcomes Educate patients and their families on injury prevention, home exercises, and community resources for ongoing care Coordinate discharge planning and facilitate smooth transitions from inpatient or hospital settings to outpatient or home health environments Maintain accurate documentation of treatments, progress notes, and care plans within electronic medical record (EMR) systems Skills Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice Proven experience in outpatient clinics Skilled in developing comprehensive care plans for diverse populations including orthopedics and geriatrics Proficiency in EMR systems for documentation and patient management Ability to perform manual therapy, orthopedic techniques, and other physiotherapy modalities effectively Excellent patient care skills with a compassionate approach to treatment and education Familiarity with discharge planning processes and community-based care coordination Join our team to empower patients through expert physiotherapy care!
