Job Description
Patient Access & Care Coordination Specialist - FT Join the Haxtun Health team as a Patient Access & Care Coordinator Specialist, where you'll play a vital role in guiding patients through their care journey—from check-out to follow-up services. In this dynamic, patient-facing role, you'll coordinate appointments, referrals, and behavioral health services while collaborating across departments to ensure seamless, timely, and compassionate care delivery.

$21.50/hr - $27.95/hr, depending on experiencePosition Status:

Provides comprehensive administrative and care coordination support to ensure a seamless patient experience from check-out through follow-up care. This role is responsible for scheduling appointments, coordinating referrals, and supporting behavioral health services, while ensuring timely completion of provider-ordered labs, imaging, and specialty care. The position also assists patients with understanding care plans, insurance coverage, and financial assistance options, working closely with billing and revenue cycle teams to ensure accurate information and support. Serving as a key liaison between patients, providers, and internal departments, this role promotes efficient clinic flow, continuity of care, and timely access to services.Qualifications:
Minimum Qualifications:
High school diploma or equivalent required Minimum of 1 year of experience in a healthcare setting Familiarity with electronic health records and insurance portals. Ability to learn health insurance processes, including prior authorization and coverage requirements. Excellent organizational and time management skills with attention to detail. Strong verbal and written communication skills. Ability to work independently and manage multiple tasks simultaneously. Compassionate, patient centered approach with sensitivity to financial and social stressors.Preferred Qualifications:
Postsecondary education in healthcare administration, social services, or a related field, preferred. Prior authorizations, billing, or patient access experience, preferred. Experience with Medicaid and other public assistance programs, strongly preferred. Benefits atHaxtun Health:

Paid Time Off (PTO): Accrued at approximately 17 days for the first year of employment for hourly employees and 22 days for the first year of employment for salaried employees. PTO accrual increases as your length of service increases. This includes holidays.Sick Leave:
Up to 48 hours per year.Retirement Plan:
Up to a 3% employer match, which increases with your length of service. New employees are automatically enrolled at a 1% contribution rate (can be opted out). Employees must complete 5 years of employment to be 100% vested.Employee Health Payment Match:
