Patient Care Liaison (In-Person) We're looking for a high-performing Patient Care Liaison who knows how to turn relationships into results! This is not a sit-back-and-manage role. This is for someone who thrives in the field, understands the pace of hospital discharge planning, and wants to own their market, drive census, and be recognized for it.
