Patient Navigator -
Community & Social Health Status:
Full-Time, Non-Exempt Pay Range:
Starting at $24.15/hour , commensurate with job skills and experience

What We're Looking For Required:
• High School Diploma or GED At least 2 years of experience in health or social services Strong knowledge of community resources Valid Hawaiʻi driver's license, insurance, and reliable transportation
Preferred:
