Field Patient Care Coordinator (Commission-Based) We are seeking a Field Patient Care Coordinator to work directly with patients in a community-based setting. This role focuses on patient engagement, education, and support related to weight management and wellness programs. Responsibilities Visit assigned patient locations in the field Communicate with patients in a professional, supportive manner Provide basic education and guidance on weight loss and wellness programs Explain program options and help patients understand next steps Document patient interactions and report updates to the care team Maintain professionalism and respect for patient needs and mobility limitations, including patients who may require wheelchair assistance or additional support Compensation This is a commission-based role tied to pre-paid patient enrollments or completed program cases Pay is structured as compensation for patient education, enrollment support, and completed engagements Full details of the commission structure will be provided during the interview process Requirements Comfortable working in a field-based, patient-facing role Strong communication skills and ability to build trust with patients Reliable transportation and ability to travel locally Ability to work independently No medical license required, but healthcare, wellness, or customer-facing experience is helpful Additional Information This role is focused on patient education and engagement, not clinical diagnosis or treatment. It is ideal for individuals who are comfortable speaking with patients, explaining programs clearly, and supporting lifestyle-based wellness goals.
