Essential Duties:

Facilitate linkages/coordinate services with outside agencies for patients with unmet needs (i.e., childcare, transportation service, psychiatry, housing, etc.) Track attendance of newly enrolled patients to ensure they complete orientation within allotted days of admission Review FGC program policies in group or individual sessions Compile data to present to medical team, in conjunction with clinical staff input, when doctor intervention is indicated Attend to patients in crisis at the request of medical reception or other staff

Monday - Friday (8:00am - 4:30pm)

Pay Rate:

Up to $18.00 per hour; based on experience Family Guidance Centers, Inc. is a Drug Free Environment (including THC and/or CBD). DCFS clearance required. High School Diploma / GED required; Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.

Certificates, Licenses, Registrations:

CADC or CPRS preferred
