- The Medication Management Pharmacy Intern is to provide support to team pharmacists, patients, and clinicians to ensure effective, safe, and cost-effective drug therapy.
- Qualifications
Education Requirements:
________________________________________________________- Minimum first professional year completed at an accredited college of pharmacy.
- Experience Requirements
- Previous community pharmacy experience preferred. English proficiency, oral and written.

- Valid New York State Pharmacy Intern Permit required. Non-Bargaining Unit, 600 - HSO Pharmacy - MSH, Mount Sinai Hospital
- Responsibilities
- 1.
Data entry:
such as obtaining medication histories directly from patients and scheduling visits with a pharmacist. Documents all patient encounters as appropriate in health system documentation system. 2.Data analysis:
such as payer data and claim 3. Support the pharmacist to implement the patient's care plan, such as navigating insurance benefits, assisting with provider communication 4. Provide medication, lifestyle, and disease state counseling 5. Coaches patients, families, and/or authorized care givers on how to use existing skills and develop new ones to make lifestyle behavior changes that can positively affect patient's health. Engages them in the use of MyChart to facilitate electronic access of patient's health care information. 6. Obtain medication lists and engages with patients about medication adherence to identify and resolve patient adherence barriers that do not require a clinical intervention 7. Notify the pharmacist/provider of a missing prescription 8. Notify the pharmacist/provider of a request for a medication refill 9. Call pharmacies to confirm prescriptions were received and picked up 10. Call patients to remind them to pick up their medication 11. Request records from clinics and facilities when necessary 12. Send referrals to other disciplines to coordinate care when necessary 13. Enroll patients into criteria-based pharmacy programs and document in the electronic medical record using established formats in a timely and accurate manner 14. Other duties as assigned- About Us
