What You'll Do Conduct inhome assessments and create individualized care plans
Provide clinical oversight, coaching, and support to caregivers and field nurses
Ensure compliance with LHCSA and NYS DOH standards through accurate, timely documentation
Collaborate closely with families, providers, and interdisciplinary team members
Support staff education and quality improvement initiatives What You Bring Active, unrestricted RN license in York (BSN preferred)
Minimum 2 years of nursing experience, including at least 1 year in home care (geriatrics preferred)
Strong clinical assessment, organizational, and leadership skills
Excellent communication and timemanagement abilities
