Palo Alto, CA Key Responsibilities:

Deliver clinical pharmacy services to an inpatient pediatric setting, ensuring safe and effective medication use Collaborate with healthcare teams to optimize therapeutic outcomes for pediatric patients Utilize EPIC software effectively for medication documentation and management Support patient care through medication review, monitoring, and consultation

Desired Experience and Qualifications:

Licensed pharmacist with hospital inpatient and clinical pharmacy experience Familiarity with pediatric inpatient care is highly preferred Experience with EPIC or similar electronic medical record systems PGY1 residency training or equivalent experience valued, with a strong work ethic Ability to adapt to variable shifts and work effectively within a collaborative multidisciplinary team
