EDUCATION
Graduate of an accredited school of pharmacy with BS or Pharm D.D. Licensure from the State of Texas Board of Pharmacy and evidence of annual renewal.
WORK EXPERIENCE
Two years of experience in pharmacy.

Ability to understand the physical, psychosocial and spiritual concerns/problems of the terminally ill and the related pharmaceutical needs. Ability to advise Medical Director and staff regarding pain and symptom management pharmaceutical options. Ability to interpret and understand the Texas Pharmaceutical Laws and Regulations. Ability to work effectively within an interdisciplinary team. Ability to relate well with patients, families, community at large and health care professionals. Ability to communicate effectively both orally and in writing. Demonstrates the ability to problem solve. Ability to be alert and awake when at work. Demonstrate competency as defined. Ability to advance in computer knowledge.
ESSENTIAL JOB FUNCTIONS
Establishment of specifications for procurement and storage of all drugs, chemicals and biologicals. Participation in the development of a formulary for the facility, subject to approval of the Medical Directors and the QAPI committee. Procurement of drugs for the inpatient facility and maintain appropriate inventory and stock levels of pharmaceuticals by monitoring stock on hand. Maintaining and making available a sufficient inventory of antidotes and other emergency medications in the inpatient pharmacy as well as current antidote information, telephone numbers of regional poison control center and other emergency assistance organizations. Oversees the drug control systems including receipt of prescriptions, storage of medications, preparations of drugs, labeling of prescriptions, preparing of drugs for distribution and dispensing of medications prescribed by a physician. Provides 24-hour availability of drugs and biologicals. Records and maintains records of all transactions of the pharmacy as may be required by applicable state and federal law, and as may be necessary to maintain accurate control over and accountability for all pharmaceutical materials. Instructing patients and caregivers regarding specific drug therapy. Provides information regarding the safe and appropriate use of drugs to other health professionals. Identify appropriate outcomes of drug therapy. Consults on drug therapy and coordinates with the IDT/IDG. Client identification is confirmed according to established policy. Participation in those aspects of patient care evaluation program which relates to pharmaceutical material utilization and effectiveness. Participation in the development, review and implementation of pharmacy policies and procedures. Meets all inspection and other requirements of the Texas Pharmacy Act. Maintains all appropriate records. Pharmacy services are provided in compliance with federal and state laws. Complies with all federal and state laws and regulations pertaining to controlled substances when destroying controlled substances for any reason. Acts independently and responsibly to perform job duties on a consistent basis. Pharmacist or designee available for consultation 24 hours a day. Keeps all patient/family information confidential. Adheres to the organization's standards of conduct. Will report any potential misconduct or violation of the organization's standards of conduct, federal or state laws and regulations. Participates in compliance with staff training and education programs. Adheres to the HIPAA security policies, procedures and practices at Hospice of Wichita Falls and understands that sanctions will be applied for non-compliance. Understands volunteers' roles at HOWF and attitude reflects an appreciation and support of volunteers. Behaves in accordance with HOWF policies/procedures demonstrating a professional manner in action and provides service to others in a willing and courteous way. Participates in departmental and/or agency wide Performance Improvement activities, as appropriate. Demonstrates the ability to be at work each day on time and maintain a good attendance record. Demonstrates team approach to Hospice care, equally supporting all other members of the team. Evaluates own needs for support and uses identified system to meet the need. Performs other duties as requested by supervisor. PHYSICAL AND SPECIAL DEMANDS
Requires full range of body motion, including turning, stooping and stretching. Also includes reaching to arms-length and bending from waist or side-to-side. May include squatting. Requires standing and walking for extensive periods of time. Requires corrected hearing in normal range for effective communication via telephone, face-to-face inquiries, alarms and general functions. Cognitive skills required for reasonable interpretation of assignments. Visual acuity required to read, notes and reports relative to duties. Legible handwriting for documentation and communication. Requires corrected vision in the normal range. Must be able to adapt to rapidly changing environment which may include stress and working irregular hours. Requires manual and finger dexterity for operating and adjusting various office equipment.
