Physical Therapist

Join Summa Health at Home and Hospice as a compassionate Physical Therapist serving Macedonia, Twinsburg, Aurora, Hudson, Kent, Streetsboro, and surrounding areas. This Physical Therapist opportunity offers meaningful one-on-one patient care in the home, helping patients regain independence and improve quality of life.

Schedule:

Monday-Friday | 8:00 AM-5:00 PM + 1 weekend on-call every 6 weeks

Bonus:

$2,500

Sign-On Bonus What You'll Do:

As a Physical Therapist , you'll provide skilled in-home physical therapy services, complete evaluations and OASIS assessments, develop personalized care plans, educate patients and families, and collaborate with physicians and interdisciplinary teams to support recovery and reduce rehospitalizations. The Physical Therapist will also supervise PTAs as needed and ensure timely, accurate clinical documentation.
