Educate patients and their families on treatment plans, home exercises, and injury prevention strategies. Collaborate with healthcare professionals to ensure coordinated care and effective discharge planning. Maintain accurate documentation of patient progress and treatment outcomes. Stay updated on the latest advancements in physical therapy practices and techniques. Provide home community care as needed to support patients' recovery in their own environments. Experience Proven experience in patient care within a clinical setting is preferred. Knowledge of physiology, anatomy, medical terminology, and discharge planning is essential.
