Responsibilities:

Evaluate and develop individualized treatment plans for residents. Provide skilled physical therapy services in accordance with physician orders. Collaborate with interdisciplinary teams to ensure optimal patient outcomes. Complete timely and accurate documentation. Educate patients, families, and caregivers on treatment plans and progress. Maintain compliance with all facility and regulatory standards.

Qualifications:

Graduate of an accredited Physical Therapy program. Current Pennsylvania PT License (or eligibility to obtain) Skilled Nursing Facility experience preferred, but not required.
