In this vital role, you will assess, plan, and implement personalized rehabilitation programs to help patients regain mobility, reduce pain, and improve overall quality of life. Your expertise will support a diverse patient population across inpatient hospital settings, outpatient clinics, home health environments, and community care programs. You will play a key role in delivering compassionate patient care while collaborating with multidisciplinary teams to ensure optimal recovery outcomes. Responsibilities Conduct comprehensive assessments of patients' physical conditions, including mobility, strength, and functional abilities. Develop and implement tailored care plans utilizing evidence-based physiotherapy techniques to promote healing and recovery. Perform therapeutic interventions such as trigger point therapy, manual therapy, and exercise prescriptions to address orthopedic, geriatrics, pediatrics, and other specialized needs. Manage discharge planning processes by coordinating with healthcare teams and patients to ensure seamless transitions from hospital to home or community settings. Document patient progress accurately using electronic medical record (EMR) systems and communicate updates to relevant team members. Educate patients and their families on injury prevention, self-care strategies, and home exercise programs to foster independence. Participate in interdisciplinary team meetings to review treatment plans and adjust interventions based on patient response and evolving needs. Requirements Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction. Proven experience working in inpatient hospital environments, outpatient clinics, or home health care settings. Strong knowledge of physiology, anatomy, medical terminology, and rehabilitation techniques including trigger point therapy. Experience with discharge planning, care plan development, and patient care documentation within EMR systems. Excellent communication skills for effective patient education and interdisciplinary collaboration.
