Performance Evaluation Title:
Licensed Physical Therapist Assistant Job Summary:
Provides coordinated physical therapy to patients/clients of all age groups under the supervision of a Licensed Physical Therapist. Participates in the coordination of care.
Job Qualifications:
Education:
Graduate of an accredited school of physical therapy assistant with an Associate Degree in Physical Therapy.
Licensure:
Current State License to practice as a physical therapist assistant, or if licensure does not apply, meets one of the requirements listed in 42 CFR §484.115(i); . Current driver's license.
Experience:
One year experience as a Physical Therapy Assistant, preferred.
Skills:
Therapy skills as set forth in the Therapy Practice Act and other regulatory agencies. Good interpersonal skills. Proof of current CPR.
Transportation:
Reliable transportation and current and valid auto liability insurance.
Environmental and Working Conditions:
Works in a health care facility or patients'/clients' homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.
Physical and Mental Effort:
Ability to do heavy lifting, bending, pulling, pushing and standing. Prolonged standing and walking required. Requires working under some stressful conditions to meet deadlines and patient/client needs and to meet patient/client and family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
Essential Functions:
Self Eval / Mgr Eval Provides services that are ordered by the physician as indicated in the plan of care, as developed, delegated and supervised by the Physical Therapist. Completes appropriate documentation . Administers non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water and electrical modalities to patients/clients. Applies concepts of infection control and standard precautions in performing patient/client care activities to prevent contamination and transmission of disease.. Accepts clinical assignments that are consistent with education and competence to care for patients/clients. Uses effective interpersonal relations and communication skills to provide patient/client and caregiver education. Meets mandatory continuing education requirements. Promotes compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary. Demonstrates commitment, professional growth, and competency. Participates in the Agency's QAPI program. Participates in Agency-sponsored in-service training.
