Full Time and/or PRN Physical Therapist (PT) - In-House Rehab | Crane, TX Focused Post Acute Care Partners (FPACP) is a dynamic company with an expectation of excellence in the delivery of long-term care to every patient every day. Position Overview As a Physical Therapist (PT), you will evaluate, plan, and implement evidence-based physical therapy programs to improve residents' mobility, strength, balance, and overall functional independence. You will play a key role in interdisciplinary collaboration, patient and caregiver education, and ensuring high-quality clinical outcomes. Key Responsibilities Evaluate residents to assess mobility limitations, strength deficits, balance, and functional impairments Develop and implement individualized treatment plans focused on mobility, gait training, strength, and pain management Monitor patient progress and adjust treatment plans as needed Supervise and collaborate with Physical Therapy Assistants (PTAs) Recommend assistive devices and adaptive equipment to promote safety and independence Educate residents, families, and caregivers on treatment plans and mobility strategies Maintain accurate and timely documentation in compliance with state and federal regulations Participate in interdisciplinary team meetings and care planning Ensure compliance with all regulatory and facility policies Qualifications Active Texas Physical Therapist (PT) license In good standing with all relevant licensing boards and regulatory agencies Experience in long-term care, skilled nursing, or geriatric rehabilitation preferred Familiarity with Net Health documentation system a plus Strong communication, leadership, and clinical decision-making skills Why Join Us?
