We are currently seeking a dedicated and compassionate Physical Therapist (PT) to join our team at a well-established Skilled Nursing Facility in Live Oak, CA . In this role, you will evaluate and treat patients to help them regain mobility, improve strength, and enhance their overall quality of life.

Responsibilities:

Evaluate patients and develop individualized treatment plans Provide physical therapy treatments to improve mobility, strength, and function Monitor and document patient progress and modify treatment plans as needed Educate patients and families on exercises, mobility techniques, and injury prevention Collaborate with interdisciplinary team members to ensure comprehensive patient care Ensure all services are delivered in compliance with federal, state, and facility guidelines Supervise and guide Physical Therapist Assistants (PTAs) as appropriate

Qualifications:

Graduate of an accredited Physical Therapy program Active California PT license (or eligible) CPR certification (preferred) Previous experience in skilled nursing or long-term care is a plus Strong clinical, communication, and interpersonal skills Compassionate, reliable, and patient-focused mindset Helping patients move better, feel stronger, and live independently—one step at a time.
