General Summary:
In collaboration with an interdisciplinary team, provides therapeutic services including evaluation, treatment, and education. Monitors the treatment activities of PTA's and aides. Rotating weekend work schedule required.
Supervisory responsibilities:
After one year of experience, may act as student supervisor. Directs treatment activities of PT assistants and rehabilitation aides.
Education, Licensure, Registration and Experience:
Graduate of an approved bachelor's degree or higher program as a PT. Previous Clinical experience/rehabilitation experience preferred. State Licensure (or eligible) as a PT CPR certification
Essential Functions:
Completes evaluations of assigned patients in accordance with the established standards of care and practice. Collaborates with other members of the healthcare team to ensure evaluations are performed safely and accurately. (i.e., utilized speech therapist for identifying best modes of communication for aphasic patient). Short
  • and long-term goals are realistic, measurable, and address functional needs.
Goals are written upon completion of evaluation per department standards and guidelines. Goals identified in collaboration with patient, family and other involved team members. Independently selects and performs therapeutic techniques that are consistent with the patient's problems and medical status and reflect an integration of the therapist's clinical knowledge and theory. Demonstrates knowledge about equipment/materials utilized in patient care per supervisor's observation and chart documentation. Provides for patient/family educational needs. Therapist adheres to all procedures and documentation standards per rehabilitation area/department. Is responsible to work within the guidelines and authorizations provided by the individual's insurance coverage, and or Medicare guidelines. Demonstrates good time management as evidenced by completing work assignments on time, readily accepting assignments and consistently striving to be a productive member of the institution and always puts time to good use. Demonstrates willingness to provide coverage in other rehabilitation areas.

Standards of Employment:
Attends all mandatory education programs and can describe his or her responsibilities related to general safety and regulatory compliance. Appearance is professional, neat, clean, and appropriate for the work environment. Wears proper photo identification badge while on duty. Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management. Attends monthly staff meetings and participates in monthly rounding process with manager to assure effective and timely two-way communications. Participates in and completes required department specific education and training programs and can describe his or her responsibilities related to department safety and specific job-related hazards. Follows safety and security policies including exposure control plans/blood borne and airborne pathogens.
