Home Health Physical Therapist (PT)•

Full-time & Per Diem Service Area:

South Monterey County | Salinas to King City Region At InHome Healthcare Services, we are committed to delivering quality care where it matters most—in the comfort of our patients' homes. As we continue expanding throughout South Monterey County, including Salinas to King City , we are seeking a compassionate and skilled Physical Therapist to join our dynamic Home Health team. We pride ourselves on building meaningful connections with our patients, especially the seniors in our communities. Our approach to care is rooted in dignity, empathy, and respect, and we are looking for professionals who share that same passion.

Qualifications:

Current California Physical Therapist license in good standing Minimum 1 year of Physical Therapy experience (Home Health or Hospice preferred, but not required) Strong communication, clinical documentation, and time management skills Valid driver's license and reliable transportation for daily field visits Current CPR certification and TB clearance

Preferred Qualifications:

Experience in Home Health or Hospice settings Ability to work with diverse patient populations and tailor treatment plans accordingly Why Join InHome Healthcare Services? Key Responsibilities:

Provide in-home physical therapy services and perform patient evaluations Monitor and document vital signs, mobility, functional status, and therapy progress Develop and implement individualized treatment plans Recommend therapeutic interventions, assistive devices, and safety modifications Collaborate with RNs, OTs, physicians, and other care team members Educate patients and caregivers on strategies to improve mobility, safety, and independence
