  • Full-time Job Responsibilities for PRN Physical Therapist in
Home Health Care Setting:
Assessment and Evaluation:
Conduct comprehensive assessments of patients' physical abilities, functional status, and mobility within the home environment. Perform ongoing evaluations to monitor progress and adjust treatment plans as needed.
Treatment Planning:
Develop individualized treatment plans based on assessment findings, patient goals, and physician recommendations. Collaborate with interdisciplinary team members to ensure coordinated care and optimal outcomes.
Therapeutic Interventions:
Administer skilled physical therapy interventions aimed at improving mobility, strength, balance, and coordination. Utilize evidence-based techniques and modalities to address functional limitations and promote independence.
Patient Education:
Provide education to patients and their caregivers on therapeutic exercises, activity modifications, safety precautions, and adaptive equipment usage. Empower patients to actively participate in their rehabilitation process and promote self-management strategies.
Documentation and Reporting:
Maintain accurate and timely documentation of patient assessments, progress notes, treatment plans, and outcomes using electronic medical record systems. Communicate effectively with the interdisciplinary team, including physicians, nurses, and other therapists, to ensure continuity of care.
Safety and Compliance:
Adhere to regulatory standards, professional guidelines, and agency policies to ensure safe and ethical practice. Implement infection control measures, maintain confidentiality of patient information, and promote a safe environment for therapy sessions.
Quality Improvement:
Participate in quality improvement initiatives and clinical reviews to enhance the delivery of physical therapy services and achieve positive patient outcomes. Identify areas for improvement and contribute to interdisciplinary team meetings and case conferences.
Professional Development:
Pursue ongoing professional development opportunities to enhance clinical skills, stay abreast of current trends and research in physical therapy practice, and maintain licensure and certification requirements.
Customer Service:
Demonstrate professionalism, empathy, and respect in interactions with patients and their families. Foster therapeutic relationships built on trust, compassion, and cultural sensitivity to facilitate optimal patient care experiences.
Emergency Response:
Be prepared to respond effectively to emergency situations, including falls, injuries, or changes in patient condition. Collaborate with the interdisciplinary team to implement appropriate interventions and ensure patient safety and well-being
