What You'll Do:

Ability to perform therapy evaluations, routine visits, and OASIS documentation Work with other disciplines to give the best care needed for each patient. Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed. What We Are Looking For:

Must have current Indiana PT license in good standing. Computer skills; must be able to work within EMR system. Reliable auto and current auto insurance. Current CPR.
