What You'll Do:

Provide one-on-one physical therapy to adult patients in their homes. Perform evaluations, develop treatment plans, and deliver skilled interventions. Adjust care plans based on patient progress and functional goals. Coordinate directly with patients to manage visit scheduling. Complete documentation using our user-friendly EMR system. Maintain compliance with Medicare guidelines and company standards.

What We're Looking For:

Licensed PT in New York State. Experience with adult or geriatric populations (home care experience a plus, not required). Strong communication skills and ability to build patient rapport. Comfortable working independently in a home-based setting. Basic familiarity with EMR systems (training provided). About the
