Duties Assess patients' physical conditions by reviewing medical histories and conducting comprehensive physiotherapy evaluations. Develop and implement tailored care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities to promote healing and mobility. Collaborate with multidisciplinary teams to coordinate patient care, ensuring smooth transitions from inpatient to outpatient or home & community care environments. Educate patients and their families on injury prevention, rehabilitation exercises, and post-discharge care instructions. Manage documentation efficiently using Electronic Medical Record (EMR) systems, including progress notes, treatment plans, and discharge summaries. Participate in discharge planning by assessing patient needs for ongoing therapy or home health services. Monitor patient progress regularly and adjust treatment strategies to maximize recovery outcomes. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice. Proven experience in acute care hospital settings with familiarity in inpatient rehabilitation processes. Skilled in creating comprehensive care plans for diverse populations such as geriatrics, pediatrics, orthopedics, and those requiring post-surgical recovery. Experience with home & community care environments and outpatient clinics to support seamless patient transitions. Proficiency in EMR systems for accurate documentation and efficient workflow management. Knowledge of discharge planning procedures to facilitate safe patient transitions from hospital to home or community settings. Pay:

$85,000.00 - $105,000.00 per year
