Job Description

Job Summary Join our dynamic healthcare team as a PRN (as-needed) Physical Therapist, where you will play a vital role in restoring mobility, reducing pain, and improving the quality of life for diverse patient populations. This flexible position offers the opportunity to work in our skilled nursing facility. You will develop and implement personalized care plans, utilizing your expertise in physiotherapy and anatomy to deliver exceptional patient care. Your proactive approach and passion for rehabilitation will make a meaningful difference in patients' recovery journeys. Duties Conduct comprehensive assessments of patients' physical conditions, including mobility, strength, and functional abilities, utilizing your physiology and anatomy knowledge. Develop and modify individualized care plans that align with patient goals and medical directives, ensuring optimal recovery outcomes. Deliver hands-on physiotherapy interventions such as trigger point therapy, orthopedics-focused treatments, and geriatrics-specific exercises to promote healing and mobility. Assist in discharge planning by coordinating with healthcare teams and providing education on home & community care strategies to support ongoing recovery. Document patient progress accurately within electronic medical record (EMR) systems, maintaining detailed records of treatments provided and patient responses. Collaborate with multidisciplinary teams. Educate patients and their families about treatment procedures, injury prevention techniques, and physiotherapy exercises to empower self-care at home. Qualifications Valid licensure or certification as a licensed Physical Therapist (PT), with current registration where applicable. Proven experience in acute care hospital environments, outpatient clinics, or home health settings demonstrating adaptability across diverse clinical scenarios. Strong knowledge of medical terminology, physiology, anatomy, and orthopedic rehabilitation techniques. Experience in discharge planning, care plan development, and implementing therapy interventions such as trigger point therapy for musculoskeletal conditions. Familiarity with EMR systems for documentation and patient record management. Background in pediatrics, geriatrics is highly desirable.
