need of PRN Home Care Physical Therapists and Physical Therapy Assistants in Manatee County. The Therapist will manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs.

Job Duties Include:

Provide in-home medical treatment following the physician's order for Therapy services Provide initial Therapy assessment and develop client's individual Plan of Care as defined by the physician's plan of treatment Administers therapy according to the established plan, utilizing appropriate procedures, equipment and techniques Re-evaluates patients, assesses their progress and makes recommendations regarding discharge Supervise PTA activities appropriate to individual patient care requirements and in accordance with company policy. Communicate changes in client's condition or other issues. Documentation of all visits and communication with the clients through the company's software program

Qualifications:

Bachelor's, master's, or doctorate degree from an accredited Program Valid License in the state of Florida 1-2 Years Experience as a Home Health PT Preferred Current CPR and TB Test or Chest X Ray Level II Background Screening (https:/info.flclearinghouse.com)
