$80,000- $123,000 ANNUALLY

QUALIFICATIONS
Graduate of an accredited Physical Therapy program New Jersey Physical Therapy License Effective communication skills Strong ethical and moral character referencesRequirements General:
Facilitates and implements current PT treatments Complies with HIPAA, OSHA and Red Flag Rules Complies and follows Medicare and other third-party payers' guidelines and rules Provide PT care that complies with organization's best practices, standards, policies and procedures Complies with both federal and state guidelines, including all licensing requirementsDirect Patient Care:
Evaluation and re-evaluation of patientAssessment :
Interprets evaluation data, history, and applicable medical documentation to determine rehabilitation potential and potential benefit from intervention; Identifies physical, social, and psychological needs of the patient, family, and significant otherGoal Setting :
Utilizes patient/family/care giver input in setting functional rehabilitation goals; Documents goals in functional, measurable terms with stated time framesTreatment Planning :
Incorporates information gained in evaluation and assessment to determine appropriate, specific treatment plan for intervention and involves others when appropriateTreatment Intervention:
Demonstrates competence in performing all procedures and treatments required in area of practice demonstrates awareness of continuity of patient care by following established plans; modifies patient care based on continual assessment of the patient and effect of treatment intervention; demonstrates skills in identifying, interpreting, and incorporating patient's psychological, physical, and social responses to intervention; provides and/or recommends equipment or supplies for home use; and provides for patient's personal care and environment throughout the treatmentPatient Education:
Provides education appropriate to each patient's level of understanding regarding evaluation, treatment, and self-responsibility and instructs in home treatment program to facilitate rehabilitation and promote independence in care as appropriateDischarge Planning:
Collaborates with patient, caregivers, and other health care professionals in planning discharge from rehabilitation servicesCommunication:
Confers and communicates with referring physicians and other appropriate persons through the use of progress notes, letters, evaluations, team meetings, and direct contact concerning the proposed course, progress, and discharge plan of each patient- OrthoNJ, LLC is required by New Jersey law to include the salary range for this role. The hiring range for this role is $80,000
