The Physical Therapist in a Home Health setting is responsible for the evaluation of patients in relation to home assessment, functional status and mobility, safety awareness and other physical therapy needs based on medical necessities. The Physical Therapist is responsible for collaborating with physician and medical care team in evaluating patient's level of function, determining treatment plan, provide skilled intervention as well as supervise personnel to ensure treatment goals are achieved to maximize and restore their patient's functional skills to the fullest in their home setting.
