//www.exploreminnesota.com The Physician Assistant is a member of the health care team who provides comprehensive health care services to patients. The PA engages in independent decision making to provide health care services to individuals throughout their life span. The APRN provides physical and psychological health assessments, formulates diagnosis, orders and interprets diagnostic procedures, prescribes appropriate treatments, manages acute and chronic conditions, and provides patient education and counseling, ensuring continuity of care and respect for individuals in the communities served. The PA may also perform minor surgical procedures within their scope. Responsibilities Systematically assesses patients' health status, arrives at appropriate diagnosis(es), and implements care plan for effective clinical outcomes. Coordinates and facilitates the delivery of care to patients within a defined area of medicine as part of a team-based care framework. Performs and manages patient interviews, physical examinations, diagnostics, and preventive health measures. Develop and manage treatment plans and prescribe medication, when appropriate. Make appropriate referrals for evaluation and/or treatment, as needed for patient care. Responsibly prescribe pharmacologic and non-pharmacologic agents and interventions as appropriate for each patient. Collaborates with the patient and the health care team members in developing realistic care expectations to set and achieve their healthcare goals, respecting the patient's autonomy, cultural, psychological, and social needs to address determinants of health. Performs appropriate medical procedures within scope of practice as defined by MHFV Credentialing and Privileging process. Complete and maintain proper and timely documentation of all patient care encounters and maintain adequate knowledge and application of FVC billing policies and procedures in accordance with organizational standards. Communicate effectively and timely with patients, approved support people, and all members of the care team. This can include, but is not limited to, triaging call, electronic messages, completion of forms, addressing referral needs and hospital transfers. Facilitates care planning as appropriate for the patient and care team needs, to include patient/support person education, managing an appropriate length of stay, proper utilization of healthcare resources, follow up planning and discharge orders/prescriptions/forms. Fulfill work expectations including working appropriate patient contact hours, completing employment tasks and administrative functions as appropriate for role, reading and responding to organizational communications, completing work task within organizationally determined standards. Contributes to the progress of system initiatives through meeting and committee participation, supporting process improvement initiatives, striving to expand patient care access, driving quality outcomes, and support a culture of patient focused team-based innovation of care delivery. Required Qualifications Graduate of an accredited PA or APRN training program. Computer and keyboarding skills Demonstrated leadership, teaching, evaluation, and communication skills. Current licensure with the Minnesota Board of Medical Practice for PA or Board of Nursing as an APRN Certification by the National Commission for the Certification of Physician Assistants or a national nurse certification organization acceptable to the MN Board of Nursing to practice as a Clinical Nurse Specialist (CNS), Certified Nurse Practitioner (CNP) Meets threshold criteria/qualifications for Credentialing and Privileging. The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Qualifications:
Graduate of a school or nursing and accredited Family, Adult Gerontology, Adult Gerontology Acute Care Nurse Practitioner training program BC/BE through the AANP or ANCC Licensed or ability to obtain APRN license with the Minnesota Board of Nursing Ability to obtain and /or maintain DEA certification for the State of Minnesota Meet threshold criteria/qualifications for Credentialing and Privileges Inpatient experience preferred
