Rather than seeing patients in isolation, you'll work closely with nurses, caregivers, families, therapists, and physicians to create continuity, improve outcomes, and help patients receive the care they need at the right time. Twin Cities Physicians continues to expand throughout Minnesota by building strong clinical partnerships and delivering care where patients need it most. Serve as a key clinical decision-maker across multiple senior communities. Help coordinate care among physicians, nursing teams, therapists, families, and community partners. What You'll Be Doing Provide on-site primary care and rounding services throughout assisted living, memory care, and long-term care communities. Perform assessments, chronic disease management, preventative care, and follow-up visits. Develop and adjust individualized care plans. Coordinate care with physicians, nurses, therapists, caregivers, and family members. Support transitions of care and help reduce avoidable emergency room visits and hospitalizations. Ensure continuity of care across a diverse patient population. Qualifications Active Minnesota Physician Assistant (PA-C) license in good standing. Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, post-acute care, or related settings preferred. Strong communication skills and confidence working collaboratively with interdisciplinary teams. Valid driver's license and reliable transportation.
