Job Description

  • 275K base PLUS Productivity bonus based on RVUS and experience
  • BC/BE Family Medicine or Internal Medicine
  • Preferred start date of August 2026
  • See patients in the SNF setting
  • Full-time permanent
  • Monday thru Friday 40-hour schedule.
  • No weekends
  • No nights
  • No phone call
  • NP will take call
  • To see 25 patients per day
  • RVU bonus above 25 patients
  • each additional patient is paid based on the RVU-based payment model. The amount per patient increases with the level of care patient needs.
  • Sign on bonus
  • Malpractice, Health Insurance (medical, dental and vision), 401k with a 3 percent match (candidate will qualify to participate starting 2nd month of their employment), long term disability insurance, Term life insurance, relo assistance, PTO, CME.
