Primary Job Responsibilities:

Documents all patient interactions in a concise manner that is compliant with documentation requirements Visit patients in the hospital to assist with screening process for transitions of care program eligibility. Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management Assists to schedule and coordinate patient transportation, follow-up physician appointments Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports Provides excellent customer service by serving as a resource to all internal and external customers Maintains knowledge of all health plan benefits and payor requirements, Call patients as designated by PCP or clinical staff Verify patients have been seen and follow up on no show patients Assist all patients with barriers Assist patient with linkage to community resources Ensure appropriate information is communicated to clinical staff when there is variance from plan of care Coordinates other medical office assistants in outreach and report validation needs

Minimum Qualifications:

High school graduate or GED equivalent preferred More than one year of related experience in a medical setting Basic computer literacy of medical terminology Good communication and customer service skills Able to perform all Medical Office Assistant job functions and responsibilities

Required Licensure/Certification Skills:

• Valid NYS drivers' license

Preferred Licensure/Certification Skills:

M•Medium Work•Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
