Job Description

JOB SUMMARY
We are seeking a dynamic and compassionate Prevention and Case Management Specialist to join our dedicated team. This position integrates STI testing, harm reduction, and case management. You will lead community-based initiatives, including HIV/HCV and Syphilis testing and the distribution and inventory management of supplies such as Narcan and safer-use kits. By managing a diverse caseload—ranging from Opioid Treatment Program and HIV Client Services participants to justice-involved individuals—you will be part of the full continuum of care from initial assessment to community reintegration. Additionally, you will drive organizational impact by coordinating outreach events, maintaining supply logistics, and fostering strategic partnerships with community organizations to ensure seamless referral networks. This position requires the ability to treat everyone with respect, regardless of who they appear to be, and employ professional boundaries with the community and consumers. DUTIES HIV/HCV and Syphilis testing, referrals and documentation Safer drug use, sex supply and HIV/Syphilis self-test distribution and stocking Narcan education, distribution, ordering and documentation Training staff for HIV/HCV and Syphilis testing Responsible for maintaining inventory for all Prevention supplies Coordinate and lead outreach activities (evenings and weekends at times) Participate in planning and executing HIV Testing Day and World AIDS Day activities Establish and maintain meaningful contacts with external community referral sources Document community/provider contacts, including successes and challenges. Facilitate psychoeducational groups at Fairbanks Correctional Center Participation in community meeting (Housing and Homeless, Re-entry, and Prevention Alliance) Case Management (Assess, Plan, Implement, Monitor, Reassess and Document)- and document all contacts and referrals - with Opioid Treatment Program applicants/consumers, HIV Client Services, DOC inmates
PERFORMANCE EXPECTATIONS
: This person must be ethical, self-motivated, open to learning new tasks, comfortable asking questions, organized and familiar with Microsoft Office, specifically Excel. Prior experience in an office setting is preferred. This position requires working independently and as part of a team. Ability to respond constructively to feedback is imperative. Training will be provided for supply exchange, STI testing, and confidentiality protection laws. $23.00 - $26.00 per hour
