Advanced Practice Provider - Primary Care About the Role Are you an NP or PA who wants to practice meaningful, relationship-driven primary care? At Thundermist Health Center, you'll serve as an autonomous licensed provider responsible for your own patient panel — building continuity, managing chronic and acute conditions, and delivering the kind of care that makes a real difference over time. Your days will include comprehensive assessments, evidence-based treatment, patient education, and close collaboration with an interdisciplinary team. What You'll Do Patient Care Manage a panel of primary care patients, providing continuity of care through scheduled appointments and ongoing management of acute and chronic conditions. Conduct comprehensive patient assessments including health histories, physical exams, and anticipatory guidance across the lifespan. Develop, implement, and update individualized care plans based on clinical assessment, patient goals, and evidence-based standards of care. Perform point-of-care testing including rapid strep, flu, COVID, urinalysis, STI testing, and more. Perform wound care, suturing, splinting, and minor surgical procedures. Order and interpret diagnostic tests (X-ray, ultrasound, labs) and initiate appropriate treatment based on results. Prescribe medications within your scope of practice. Maintain complete and accurate same-day clinical documentation. Supervise nurse practitioner students as assigned. Collaboration & Administrative Conduct peer chart reviews on a rotating basis. Participate in departmental and team meetings to support care coordination and quality improvement. Maintain compliance with credentialing, re-credentialing, and privileging requirements. Meet patient throughput and productivity standards to support efficient, accessible care. What We're Looking For You're a confident, thoughtful clinician who brings both clinical expertise and genuine warmth to every patient relationship. You're comfortable owning a panel, managing complexity, and motivated by the mission of making high-quality primary care accessible to everyone. 3+ years of experience as an NP or PA in a primary care setting, with demonstrated experience managing a patient panel. Active Rhode Island RN and NP or PA licensure. ANA Certification in your specialty (family medicine, internal medicine, women's health, or pediatrics). Ongoing continuing education to maintain and update clinical skills. Exceptional oral and written communication skills, with proficiency in applicable medical terminology. Demonstrated commitment to patient-centered, high-quality care for diverse and underserved populations, and an authentic passion for health equity and community health. Schedule & Locations This is a full-time (40 hours/week) onsite position. 80% of your time will be patient-facing; 20% is dedicated to administrative duties, which can be completed from home or onsite. Clinic hours are Monday-Thursday, 8:00am-8:00pm, and Friday, 8:00am-5:00pm. At least one clinical day per week will include hours after 5:00pm to support patient access.
