Job Description

Role Description The Primary care physician is responsible for the coordination and oversight of medical care and quality of medical services provided to patients at the Clinic. The PNP will ensure proper functioning of day-to-day operations of the clinic in coordination with other clinical staff, to provide consistent quality of care to patients and ensure medical needs are met of all clinic patients on Urgent Care, Primary Care, and Occupational health of the clinic. Qualifications
PROFESSIONAL
Graduation from an accredited MD or DO school of medicine is required. Current license to practice medicine in the State of New York is required. Current DEA license is required. Must meet all credentialing requirements as required by Medicaid, Medicare and insurances. Must meet all credentialing requirements as required by Orleans Community Health's Medical Staff Bylaws Board eligible or Board Certified. If board eligible at time of hire, must be Board Certified within 18 months of hire date.

Must have a sense of initiative and responsibility to be a member of the health care team. Be of good moral character as determined by the department and hospital. Ability to relate to other people beyond giving and receiving instructions.
CONTACT:
Works alone, with, and around others. Talks face to face and has verbal contact with others including patients, visitors, hospital personnel, and students. Must hear normal conversation and telephone communication. Must have visual acuity form near and far, depth perception, and accommodation.
ESSENTIAL PHYSICAL DEMANDS
Lifts 35 lbs., carries, pushes and pulls frequently. Stands and walks constantly (>5 hours). Sits occasionally. Kneels, bends, and squats frequently. Crawls Reaches forward frequently, and reaches overhead occasionally. Able to hear with accommodations and have functional sight.
NON-ESSENTIAL FUNCTIONS
Ability to transport patients, clerical duties.
WORKING CONDITIONS
Exposure to disagreeable odors, communicable disease, infections and injuries from patient/equipment. Work primarily indoors. Must be able to travel between clinical sites as needed. Works with electrical and mechanical devices.
Essential Functions:
Lead the Care Team in medical decision-making; provides supervision/collaboration to NPs and PAs. Perform HPI and physical examinations. Diagnose and direct counseling for each patient on a plan for treatment. Prescribe medication in accordance with New York State statute and professional practice guidelines. Perform minor surgical procedures. Generate referral for patients requiring services outside the scope of direct primary care. Evaluates patient related data (incoming reports, telephone encounters, messages, labs, diagnostic imaging, EKG) and directs follow-up in a timely manner. Completes timely and accurate chart documentation. Adhere to Orleans Community Health Center scheduling guidelines and productivity standards. Participate in staff outreach and off-site health care programs in the Orleans Community Health service area as assigned. Comply with the Orleans Community Health Infection Control plan, which includes personally utilizing universal precautions. Ensure compliance by support staff. Regularly participates in departmental, provider, staff and other meetings as assigned. Assist in updating Orleans Community Health protocols and principles of practice as requested. Assist in outside medical public relations functions. Must maintain an unrestricted license to practice medicine in the State of New York including, but not limited to, all applicable narcotics licensure (including DEA) required for the full exercise of Physician's medical staff membership and clinical privileges at Orleans Community Health.
