JOB SUMMARY
As a Home Health Physical Therapist Assistant, you'll play a vital role in empowering patients to achieve their highest level of function and independence—all within the comfort and familiarity of their own homes. Using your clinical skills and compassionate care, you'll help individuals regain mobility, restore confidence, and improve their overall quality of life. Working under the supervision of a licensed Physical Therapist, you will assist in delivering both direct and indirect physical therapy interventions in accordance with the patient's individualized treatment plan and state practice guidelines. Your contributions will support recovery, reduce the risk of hospital readmissions, and foster long-term wellness. As a valued member of our interdisciplinary home health team, you will report directly to the Home Health Director. Regulatory Requirements:
v Graduate of an accredited program for Physical Therapy Assistants v Prior experience in the provision of physical therapy services preferred. v Experience in acute, long term care and/or outpatient therapy.
Language Skills:
v Ability to read and communicate effectively in English. v High degree of empathetic listening, tact and kindness toward others. v Ability to communicate well verbally. Must work effectively as a member of a team.
Skills:
v Strong computer knowledge v Ability to work with independence and with minimal direction.
