About Alameda Family Services Alameda Family Services (AFS) is a progressive, non-profit agency that provides clinically sophisticated, comprehensive mental health and early childhood educational and family support services to individuals and families in the City and County of Alameda.
Programs include:
Early Head Start, Head Start, Family Support Services, School Based Services, and Behavioral Health Care Services (Counseling and an APA- accredited Narrative Therapy Training Program). Position Summary The Enhanced Care Management (ECM) & Family Resource Center (FRC) Program Supervisor is a dual-focus leadership role responsible for overseeing both direct service staff and the day-to-day operations of AFS's ECM and FRC programs. The Supervisor ensures high-quality, equitable service delivery to Medi-Cal members and families with complex needs, while maintaining compliance with Medi-Cal managed care requirements and county partnership agreements. This role requires an experienced, equity-centered leader who can manage a multidisciplinary team — including care managers, and family navigators — while maintaining strong accountability structures, data integrity, and community partnerships. Key Responsibilities Program Oversight & Operations Oversee implementation and quality of the ECM program in alignment with Medi-Cal managed care plan requirements and DHCS guidelines. Supervise day-to-day operations of the Family Resource Center, ensuring accessible, welcoming, and culturally responsive services for families. Monitor caseloads, workflows, and client outcomes to ensure program fidelity, contract compliance, and equitable service access. Lead team meetings, case consultations, and cross-program coordination to promote integrated care and reduce service gaps. Maintain working knowledge of ECM eligibility criteria, billing requirements, and documentation standards. Staff Supervision & Development Directly supervise a multidisciplinary team of Care Managers, Family Navigators, and other program staff. Conduct regular individual supervision sessions, team huddles, and performance evaluations. Provide coaching, mentorship, and professional development support to build staff capacity. Foster a team culture grounded in trauma-informed care, cultural humility, and racial equity. Coordinate onboarding and training for new hires, including ECM protocols, FRC services, and EHR documentation practices. Data, Documentation & Compliance Ensure timely, accurate documentation of client services in the EHR system (Exym) in compliance with Medi-Cal and HIPAA requirements. Track program metrics, generate reports, and present data to leadership to support quality improvement and grant reporting. Participate in audits, managed care plan reviews, and county monitoring site visits as needed. Identify and escalate compliance concerns or documentation gaps promptly. Community Partnerships & Outreach Cultivate and maintain relationships with Medi-Cal managed care plans, county departments, hospitals, schools, and community-based organizations. Represent AFS at community meetings, coalitions, and stakeholder convenings. Support outreach and enrollment efforts to connect eligible Medi-Cal members to ECM services. Qualifications Required Bachelor's degree in Social Work, Public Health, Psychology, or a related field (Master's degree preferred). Minimum 3 years of experience in community health, behavioral health, or social services, with at least 1-2 years in a supervisory or lead capacity. Direct experience with Medi-Cal Enhanced Care Management (ECM) — including eligibility, workflows, and billing documentation requirements. Demonstrated ability to manage multiple priorities, lead a team, and maintain accountability to program deliverables. Familiarity with Medi-Cal managed care systems, DHCS guidelines, and care coordination frameworks. Strong commitment to racial equity, cultural humility, and trauma-informed approaches. Proficiency with EHR systems and data management; experience with Exym a plus. Excellent verbal and written communication skills. Preferred Master's degree in Social Work (MSW), Public Health (MPH), or related field. Licensed or registered in California (LCSW, LMFT, ASW, AMFT) or public health credential. Bilingual/bicultural abilities, particularly in Spanish, Cantonese, Mandarin, or other languages spoken by AFS clients. Experience with Family Resource Centers or family-centered service models. Knowledge of Alameda County's health and social services landscape.CONDITION OF EMPLOYMENT 1. A
