Job Description

PROS Licensed Mental Health Practitioner - LMSW, LCAT, LMHC, LCSW or related
Oakdale, NY Salary:
$57,296 - $60,798/yr At Family Residences and Essential Enterprises, Inc. (FREE), we empower over 5,500 people with intellectual/developmental disabilities and mental health challenges to thrive in their communities. FREE provides a diverse array of supports and services including: housing; recovery services; transition to work; employment; day, community and family services; respite; crisis services; education and after school support; specialty health services; and advocacy. The PROS Licensed Mental Health Practitioner provides direct support to program participants, including accompanying and transporting individuals to scheduled outings and services as needed. This role also includes reviewing and signing clinical documentation and ensuring all services align with NYS OMH, Federal, Managed Care, Medicaid, and COA requirements. Specifically, you will: Coordinate and implements individualized service plans, including progress tracking and periodic reviews. Manage a caseload of individuals with one or more mental health disorders. Conduct individual and group sessions to build daily living, coping, and independent living skills. Complete assessments to support treatment and service plan development. Collaborate with individuals and interdisciplinary teams to develop plans that support personal goals and outcomes. Promote and safeguards individuals' rights, health, and safety at all times. Support individuals in overcoming barriers and building meaningful relationships and social roles. Coordinate COA activities and develops curriculum to meet program needs. Provide field-based services in the community and individuals' homes as needed. Identify and reports abuse in accordance with policies. Participate in required meetings, events, and committees, including those outside standard hours. Assist with screenings, intakes, and admissions. Ensure timely, accurate, and compliant documentation (paper and electronic).

We require:
Master's degree in mental health, social work, creative arts therapy, or a related field required LMSW, LCAT, LMHC, LCSW, or related licensure required Experience in behavioral health and/or developmental disabilities field preferred Strong communication, organization, and leadership skills Ability to work effectively in a team environment with flexibility and proactive communication Computer literacy and familiarity with electronic medical records and Microsoft Office Suite required Valid driver's license and ability to meet driving requirements required Must be efficient, well organized, and able to anticipate needs We're excited to offer
