Description Job Summary:
The Protective Services / Options Care Manager is responsible for delivering comprehensive care management services to consumers in their homes while supporting Protective Services staff as needed. This role includes conducting in-home assessments, developing and implementing person-centered care plans, monitoring service effectiveness, and assisting with Protective Services Reports of Need and investigations.Qualifications:
Registered Nurse or Bachelor's degree in a related field with at least one (1) year of experience providing direct services to older adults OR High school diploma with a minimum of five (5) years of related experience Strong verbal and written communication skills Proficiency in computer systems and data entry Valid Pennsylvania Driver's License and reliable transportationEssential Functions:
Conduct in-home assessments using approved tools to determine eligibility and need for Agency services Accurately enter assessment data into SAMS or other approved systems Develop and implement person-centered care plans that address consumer needs and available support systems Coordinate and arrange services by submitting appropriate documentation to selected providers Perform ongoing monitoring to evaluate the effectiveness of services and adjust care plans as needed Conduct outreach to individuals who may be socially, physically, or culturally isolated and inform them of available services Provide counseling and guidance to consumers and families regarding care options and resources Refer consumers to appropriate community resources, including Medicare services, long-term care facilities, and support programs Maintain regular contact with consumers on waiting lists in accordance with required timelines Document all consumer interactions through timely and accurate progress notes Compile and submit monthly service reports to the supervisor Perform duties related to guardianship cases as assigned Assist Protective Services staff with Reports of Need and investigations as requested Participate in on-call rotation to receive and investigate protective service referrals Maintain knowledge of the Older Adult Protective Services Act and applicable PA Code regulations; attend all required trainings Collaborate with State Police and emergency personnel to coordinate care and ensure consumer safety Testify in court when necessary regarding protective service cases, including those involving guardianship Additional DetailsDriver's License:
