Job Description

Geode Health is a rapidly growing, national provider of outpatient mental health services. Our Mission is to "Transform mental health by making it more accessible, affordable and effective". We are looking for passionate Psychiatric Nurse Practitioners for both full and part time roles. The ideal candidate will be motivated by joining a quickly growing organization, and excited to help us improve the care experience for patients and providers. Applicants must be Psychiatric Mental Health Nurse Practitioner certified, licensed to practice, and have experience working with adult and/or child and adolescent populations.
Key Responsibilities:
Provide psychiatric care, including evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs Document and navigate EHR independently Maintain strict client confidentiality Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information Evaluate patients for the need for a higher level of care, including hospitalization Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services Work closely with Supervising physician to ensure improved patient outcomes and high-quality care Diagnose and treat patients from different backgrounds/diverse backgrounds in a respectful manner Document and navigate EHR independently and in a timely manner. Work collaboratively with clinical leadership, billing, and compliance to ensure accurate and appropriate documentation.
Qualifications :
Must hold current license/registration to practice Excellent verbal and written communication skills Must have the ability to exercise sound judgment and discretion Ability to maintain highly confidential information
Working Conditions:
Standard office environment with occasional travel to client locations. May involve evening or weekend hours to accommodate client schedules. Potential exposure to stressful situations and emergency scenarios.
Physical Requirements:
Ability to sit for 3-4 hours at a time between patient care and documentation. Fine finger coordination required for typing, pressing objects such as utilizing laptop/mouse for documentation purposes Ability to speak clearly with patients and team members so others can understand. Ability to hear and understand others when speaking Ability to view computer screen monitor, small objects few feet away Ability to act and move quickly in emergency situations involving patient care
