Job Description

Essential Duties and Responsibilities:
Plans, administers, and directs residential services programs for adults with developmental disabilities, living in agency CILA homes, or independently in scattered sites throughout the community. Assesses various strengths and limitations of individuals with disabilities to facilitate the development of a residential service plan. Provides counseling, training, and guidance in skills development, in personal and social skills development.
  • Treats others with respect, courtesy and tact and promote an environment that encourages diversity and inclusion
  • Develops Implementation Strategies and assures staff are trained and follow individual plans
  • Selects, administers, serves and interprets standardized agency approved assessments for persons served
  • Responsible for monthly review of individual progress. Prepare reports for clinical record and for interdisciplinary team
  • Provides case management, schedules and coordinates individual staffing, maintains and updates individual files
  • May assist individuals in developing techniques for selecting appropriate housing. Will serve as liaison with landlords, and utility companies if necessary.
  • Develop and coordinate emergency procedures for persons served.
  • Assist with crisis (on-call) coverage after hours, weekends and some holidays.
  • Perform related duties as assigned by supervisor
  • Maintain compliance with all agency policies and procedures

            Work Experience Requirements:
            • Computer proficiency (Vertex/Microsoft/Gmail)
            • Bachelors degree in Human Services
            • Must qualify to be certified as a Qualified Intellectual Disability Professional (QIDP)
            Physical Requirements:
            • Safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards
            • Maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
            • Must stand, walk, sit, reach with hands and arms; stoop, kneel, crouch, or crawl, and lift and carry up to 50 lbs
            • Must be able to talk, listen and speak clearly in person and on the phone
            • Specific vision abilities required by this job include close vision, distance vision and peripheral vision
