We are seeking Qualified Mental Health Professionals for o ur ______ Waverly Nursing Home Pilot Program ______________ department. •Gero-Psychiatric, Mental Health, or ID/DD experience is preferred for this position. •Responsibilities and Duties for
QMHP:
Teach consumers appropriate strategies to manage behavioral and mental health symptoms Assist with consumer service plans related to identified needs and goals Assist consumers in gaining access to appropriate medical, psychiatric, social, educational, vocational and other support and services Provide consumer training/reinforcement, linkage to supports/services Advocate on behalf of consumer, family and providers Monitor/assess service delivery Behavior management and crisis intervention.
Qualifications and Skills:
Bachelor's Degree in a Human Service field One year experience working directly with individuals with intellectual disabilities, mental illness, substance abuse or who are dually diagnosed. QMHP-C or QMHP-A certified by the Board of Health Professions Valid Driver's License with acceptable DMV record.
