Job Description
Overview We are seeking a passionate and dedicated Register Dietitian and Nutrition Education Coordinator to join our dynamic healthcare team. This vital role combines clinical expertise with educational leadership to promote optimal nutrition and health outcomes for our residents. As a key member of our dietary department, you will oversee patient assessments, develop personalized care plans, and lead nutrition education initiatives. Responsibilities Provides counseling for participants with high-risk conditions. The NE coordinator is responsible for planning, creating, implementing, and evaluating NE at the Local Agency. Conduct comprehensive patient assessments, including dietary history, physiological status, and diagnostic evaluations to inform tailored nutrition care plans. Develop and implement individualized diet plans that address specific health conditions such as assisted living needs, tube feeding requirements, and chronic disease management. Provide expert guidance on dietary management strategies, including tube feeding protocols and care for residents with complex medical needs. Lead nutrition education sessions for residents, families, and staff to promote healthy eating habits and lifestyle modifications through engaging health coaching. Supervise and mentor dietary staff, ensuring adherence to best practices in dietetics, safety standards, and regulatory compliance. Oversee the dietary department's operations, including budgeting, inventory management, and quality assurance initiatives. Collaborate with interdisciplinary teams to update care plans based on ongoing assessments and evolving health conditions. Qualifications Registered Dietitian (RD) credential with current licensure or certification as required by local regulations. Proven experience in dietary management within assisted living or long-term care settings. Strong knowledge of physiology, nutrition science, and diagnostic evaluation techniques. Demonstrated leadership skills with supervising experience in a healthcare or dietary department environment. Familiarity with patient assessment procedures, care planning processes, and health coaching methodologies. Experience managing budgets and overseeing departmental operations effectively.
