Job Description

Position Summary:
The Registered Nurse (RN) is responsible for overseeing the clinical and health-related services of residents in accordance with their individualized service plans, state regulations and community policies. The RN ensures quality care, supervises delegated medication administration, and supports a safe and person-centered environment.
Key Responsibilities:
Resident Health Oversight Complete health assessments and participate in developing and reviewing Individualized Service Plans (ISPs). Monitor resident conditions and communicate changes in health status to physicians, families, and care teams. Oversee implementation of health-related services, including wound care, diabetic care, and fall prevention. Participate in care conferences and multidisciplinary reviews. Medication Management & Delegation Delegate and supervise caregiver staff in accordance with state guidelines medication administration. Conduct medication audits and ensure MAR accuracy and medication compliance. Provide training and ongoing competency evaluations for caregivers administering medications. Compliance & Documentation Ensure compliance with state guidelines, HIPAA, and infection control standards. Maintain complete, accurate, and up-to-date nursing documentation. Support state survey readiness and participate in mock audits, quality reviews, and corrective action planning. Training & Team Support Provide training and support to caregivers on health-related topics, including chronic condition management, documentation, infection control, and emergency procedures. Collaborate with the Administrator and Life Enrichment Coordinator to support resident wellness programs.
Qualifications:
Current and unencumbered Registered Nurse license in the State of Wisconsin. Minimum 1-2 years of nursing experience preferred; experience in assisted living, long-term care, or geriatrics highly desirable. Knowledge of state regulations or willingness to become familiar upon hire. Strong leadership, clinical judgment, and communication skills. CPR/First Aid certification required or must be obtained within 90 days of hire.
Working Conditions:
May include on-call responsibilities Must be able to lift up to 30 lbs and assist with resident transfers in emergencies. Requires walking, standing, and occasional travel between resident rooms and facility departments.
