Job Overview MCO Homecare Services LLC is seeking a dependable, compassionate, and experienced Registered Nurse — RN Field Supervisor / Home Care Nurse to join our home care team in Douglasville, GA 30135 and surrounding West Georgia / Metro Atlanta communities . The RN will provide professional nursing oversight in a home care setting. Duties may include completing initial client assessments, reassessments, home supervisory visits, caregiver evaluations, caregiver skills training, medication management oversight, care/service plan development, chart reviews, documentation reviews, and quality assurance support. This position is ideal for an RN who is organized, detail-oriented, professional, and comfortable working independently in client homes while helping ensure clients receive safe, appropriate, and person-centered care. Responsibilities The RN Field Supervisor / Home Care Nurse may be responsible for: Complete initial client assessments to determine care needs and level of care required Complete reassessments, observations, and follow-up visits as assigned Develop, review, and update client care/service plans in coordination with the client, responsible party, physician, and other healthcare providers as applicable Perform home supervisory visits to monitor quality of care, client safety, and caregiver performance Observe and document changes in client condition, including improvement, decline, new symptoms, medication concerns, and new care needs Review medications, medication reminders, medication-related concerns, and medication management needs within RN scope of practice and agency policy Educate clients and/or responsible parties regarding care needs, safety, health concerns, and service expectations Train, supervise, coach, and support direct care workers/caregivers verbally and in writing Complete caregiver competency evaluations, annual caregiver evaluations, and PCA skills testing for new caregivers as required Review caregiver care logs, progress notes, service records, and required documentation Assess client and employee safety risks and recommend preventative measures Communicate with physicians, healthcare providers, clients, families, caregivers, office staff, and administrators as needed Assist with care coordination between MCO Homecare Services LLC and other healthcare providers when necessary Maintain accurate client progress notes, clinical notes when applicable, and required agency records Assist with chart reviews, documentation audits, quality assurance reviews, and compliance checks Report client progress, problems, incidents, complaints, suspected abuse, neglect, exploitation, or serious incidents immediately according to agency policy Respond appropriately to emergencies, including calling 911 for medical emergencies and notifying the office/supervisor as required Maintain client confidentiality and comply with HIPAA, client rights, and agency privacy policies Submit all required documentation within 24-48 hours after completing visits or assigned tasks Comply with agency policies, procedures, state regulations, federal laws, OSHA procedures, and universal infection control practices Participate in special projects, surveys, studies, policy updates, or quality improvement activities as assigned Perform other duties as assigned within RN scope of practice and the home care setting Requirements / Qualifications Current and active Georgia RN license required Graduate of an accredited nursing program required BSN or MSN preferred, but not required unless specified by assignment Current CPR, First Aid, or BLS certification required At least 1-3 years of nursing experience preferred , preferably in home care, home health, long-term care, geriatric care, community-based care, or case management Experience completing client assessments, reassessments, supervisory visits, or care plans preferred Experience training, supervising, or evaluating caregivers preferred Experience with medication management, medication reconciliation, or medication oversight preferred Must have strong organizational, supervisory, documentation, and communication skills Must be computer literate and able to complete electronic documentation Must be dependable, professional, compassionate, non-judgmental, and detail-oriented Must be able to work independently with or without direct supervision Must be able to think and act quickly in case of an emergency Must understand and follow client rights, confidentiality, HIPAA, infection control, OSHA procedures, and agency policies Must be able to complete and submit documentation accurately within 24-48 hours Must have reliable transportation, a valid driver's license, and automobile insurance Updated negative TB test required; if TB test is positive, a cleared chest X-ray is required Must report known exposure to TB or hepatitis immediately according to agency policy Must provide evidence of adequate health status to perform duties Must be able to pass a background check Bilingual is a plus, but not required Physical Requirements This position may require the ability to stand, sit, walk, bend, kneel, crouch, climb, stoop, push, pull, lift, assist clients with mobility needs, communicate clearly, and work safely in client homes and community-based environments.
