This role offers the opportunity to build meaningful relationships with participants while providing attentive, individualized care in a supportive, community-based setting. Responsibilities Perform daily nursing assessments, including vital signs and monitoring for changes in condition Develop and update individualized care plans and ADL documentation Assist with and oversee activities of daily living (ADLs) as needed Maintain accurate and organized clinical documentation Communicate with physicians' offices to obtain medical records and coordinate participant care Provide patient and family education on health, safety, and ongoing care needs Collaborate with families and interdisciplinary team members Respond to changes in condition and ensure participant safety Qualifications Active, unrestricted Registered Nurse (RN) license in the state of Massachusetts Prior experience in adult day health or long-term care (preferred) Strong clinical assessment, critical thinking, and organizational skills Proficiency in computer-based documentation, including completing forms and maintaining accurate, well-organized digital records Excellent communication and interpersonal skills, with the ability to collaborate effectively with patients, families, and interdisciplinary team members Demonstrated commitment to compassionate, patient-centered care A genuine interest in building meaningful connections and being an active, valued part of a supportive care community
Pay:
$38.00 - $45.00 per hour
